Public Health Plagued with Triple Burden – Thinking, Leadership & Implementation

Public health, though etched in history since antiquity, has never held such profound weight as today. As populations surge and disease evolves, the battles we fight are starkly evident in the countless lives lost to historical plagues. COVID-19, a harsh reality check, exposed the glaring inadequacies of our global public health system. Decades of neglecting prevention and health promotion have left us with a broken public health system ill-equipped for modern threats.

While disease forms mutate and new health challenges emerge, our responses remain alarmingly stagnant and the same. Read Daniel Defoe's "A Journal of the Plague Year," a chilling account of the 1664-65 Great Plague of London. Flip through its pages and witness the eerily similar narratives of fear, isolation, and rudimentary containment measures as we experienced during COVID-19 pandemic. Despite 357 years between the Great Plague of London and the COVID-19 pandemic, our progress in public health strategy appears painfully minimal.

We have embraced advancements like AI, chatbots, and machine learning, yet public health innovation languishes at a 17th-century level. The question that begs an answer: what are we missing? Why has public health progress lagged behind other scientific disciplines?

Public health has now been limited to sloganeering. Look at the past few decades, "Health for All by 2000 at Alma Ata. " Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity." -these noble mantras have echoed through public health for decades, yet the reality falls short of the promise. Are we trapped in a cycle of empty slogans, or is it time to demand leadership that delivers solutions?

Let's face it: slogans don't heal communities.

Over time, we have started to appreciate problems. Leadership is about alleviating the problems. Where are the leaders who tackle root causes with concrete action? Where are the visionaries who translate ambition into tangible progress?

We must demand a new breed of leadership, one that confronts real issues, not merely talks about them.
